ADOLESCENT HEALTH CONSORTIUM

The Adolescent Health Consortium (AHC) coordinates direct counseling and therapy services for students and families in the community of Hayward, California. The partners in this program are Vesper Society, Alameda County Office of Education/Hayward Community School, St. Rose Hospital’s Silva Pediatric Clinic, and Tiburcio Vasquez Health Center, Inc.

The purpose of the program is to assist Hayward Community School students in overcoming health-related obstacles that prevent their success in a regular school setting, by helping the students meet their basic physical and mental health needs, and helping their parents meet their own mental health needs; and to meet the physical and mental health needs of adolescents presenting to the Silva Clinic who lack resources to meet their needs.

Services are delivered at the Tiburcio Vasquez Health Center and St. Rose Hospital’s Silva Pediatric Clinic. These two local clinics provide comprehensive, culturally-sensitive mental health services, case management, direct health screening, treatment, and counseling. These services represent the first effort to provide comprehensive mental health and case management services to an adolescent population in this community.

In 2004, the AHC provided extensive outreach including more than 200 hours of individual and family counseling and therapy; nearly 300 hours of group counseling; nearly 50 hours of family group counseling; and nearly 100 hours of case coordination.

Silva Pediatric Clinic provided nearly 300 hours of health coverage access coordination and referrals for individuals and families to available healthcare coverage.
